A. Client Information Form: Please print and complete this form and bring the form with you for your first session. If you are unable to do so, a form will be provided at your session.

B. Sessions & Fees: Each session will last 50 minutes and will typically be scheduled once per week. The fee is $150 per session, payable in cash or check at the end of each session; insurance is not accepted. In the case of special circumstances, billing may be arranged and the fee may be negotiated.
